PRIVACY NOTICE - Registered with ICO

This Notice sets out your rights and how we comply with Data Protection Law concerning information we hold about you. The purpose of our data collection and retention relates to the professional service we provide to you.

Information we collect and hold
1. Your name, postal address, e-mail address, telephone numbers, details of your GP, psychological medication prescribed and details of any other relevant health professionals engaged in your care.
2. We would only contact your medical practitioner/s or others with your consent, unless we consider you or others to be at risk.
3. The record of your consultation with the clinic is retained by the clinic as you may re-present for further assistance at a later time. Correspondence with you, and with others who may be associated with your care, will also be retained.
4. Brief notes, in anonymised form, on your sessions with your therapist will be made by your therapist. This is to assist the process of your therapist’s clinical supervision and these notes will be safely destroyed after their use for this purpose.
5. A record of session attendance and payment will be held.

6. Clinic records are retained for a maximum period of 10 years.

Protecting information about you and who can see it

We make every effort to treat and store securely all information about you – on paper and held electronically – as personal and confidential.

To ensure good professional practice for you, we consult if needed with clinical colleagues within The SAP about your treatment. In this case, your personal identifying information is anonymised to protect your identify whenever necessary.

In the rare event that your therapist is unable to continue your treatment, SAP colleagues may assist and will be provided with your contact details should it be necessary for them to meet or speak with you.

Your information is only seen by clinicians involved in your treatment and by our administrators. All clinicians are subject to The SAP and BPC (British Psychoanalytic Council) Codes which require compliance with ethical standards, including respect for confidentiality. Our administrators are required to manage information confidentially as a condition of their contract.

Why we collect and hold your information


1. We only collect information relevant to your application to The Belsize Practice and to your treatment with Diana Moffat, if this follows consultation.


2. Information held relates to your therapist’s work with you and may be used for training, statistical and research purposes. If this is the case, all identifying information will be anonymised.
3. Application, consultation and treatment information and correspondence is retained by the clinic in case you re-present to the clinic for assistance at a later time. This information will help us understand your history of treatment and assist us in responding to you.

Your rights


1. You have the right to see any information we hold about you.
2. You have the right to request that information we hold about you is deleted once your treatment with the clinic has been completed.

3. You also have the right to require that information we hold about you is rectified if it is inaccurate or incomplete.


4. To access information and to request rectification or deletion, you may raise this with your therapist (Diana Moffat) or contact the administrator of the clinic in writing by e-mail to dianamoffat@icloud.com



Data management and complaints

Should you be dissatisfied with the way The Belsize Clinic (Diana Moffat) has managed your information and your complaint, you may submit a complaint to the Office of the Information Commissioner.
